Occupational Therapy in 9 countries Tap a country to see how to qualify there

5 United Kingdom Recognised, paperwork only High demand

Paperwork and a possible interview-style competence discussion, no written exam. An English test is needed.

Recognition
4/5 Mutual recognition or an assessment plus registration. No major exams.
Time to practise
9-15 months
What it takes: exams, language test, cost, regulator
Exams
None routinely. If required, a test of competence in the form of a structured professional discussion, not a written paper.
Language test
IELTS overall 7.0 with nothing below 6.5, or OET (OT) 1400 with nothing below 300, under 2 years old.
Rough cost
Scrutiny fee GBP 678.38 plus the international registration fee of GBP 370.02 (three professional years; only two years, GBP 246.68, if you join in the first year of your profession's registration cycle), excluding the English test and visa

Worth knowing: The English test catches most South Africans by surprise. HCPC fees rise most years, so re-check before budgeting.

Step by step 5 steps

  1. Get your SA university to complete and stamp the HCPC Course Information Form showing theory and practical hours per module.
  2. Sit IELTS Academic (overall 7.0, nothing below 6.5) or OET; South Africa is not on HCPC's English-exempt list.
  3. Apply online with certified documents, your practice history, HPCSA registration and a mapping of your training to the HCPC Standards of Proficiency.
  4. Two OT assessors review the file: about 3 months to a first decision, then about 3 more months of verification with your university, HPCSA and referees.
  5. If assessors want more, sit a test of competence (a structured professional discussion, usually on Teams), then pay the registration fee.

5 Ireland Recognised, paperwork only High demand

OT is on Ireland's Critical Skills list and CORU assesses your file, with no exam in most cases.

Recognition
4/5 Mutual recognition or an assessment plus registration. No major exams.
Time to practise
6-12 months
What it takes: exams, language test, cost, regulator
Exams
None routinely. An aptitude test (EUR 500) only if CORU finds substantial differences; a maximum of two attempts.
Language test
From 8 May 2026 the university-letter exemption covers only the last 2 years' graduates; else IELTS 7.0, OET B (C+ allowed in one component) or Cambridge C1 Advanced grade C.
Rough cost
Recognition application fee EUR 510 (CORU's guidance notes; its FAQ still says EUR 410), registration EUR 100, annual renewal EUR 100

Worth knowing: For non-EU applicants, offering a compensation measure is discretionary: CORU may simply refuse. The adaptation period is served in Ireland.

Step by step 5 steps

  1. Get an HPCSA letter confirming you are entitled to practise in South Africa.
  2. Apply online for Recognition of International Qualifications with certified transcripts, syllabus, placement details and stamped university forms, and pay the fee.
  3. Provide English evidence: a university letter (only if you qualified in the last 2 years), or IELTS 7.0 with nothing below 6.5, OET grade B in three components and C+ in one, or Cambridge C1 Advanced at grade C or above.
  4. Complete Garda eVetting and a statutory declaration; CORU acknowledges within a month and decides within 3-4 months of a complete file.
  5. If substantial differences are found, choose a Period of Adaptation in Ireland or an Aptitude Test (EUR 500), then register.

3 Netherlands Major requalification Some demand

Dutch at B2 is a legal requirement and the recognition file alone takes 6-18 months.

Recognition
2/5 Substantial re-study, long supervised practice, scarce posts or a hard language barrier.
Time to practise
1.5-2.5 years including learning Dutch
What it takes: exams, language test, cost, regulator
Exams
None confirmed for occupational therapists. The BI-toets knowledge test applies only to doctors, dentists and nurses.
Language test
Dutch at CEFR B2 is required by the regulator for HBO-level professions including ergotherapeut, plus English reading at B1 (for example IELTS 4.0-5.0); a diploma supplement showing the professional training was in English also counts as proof.
Rough cost
The CIBG application is currently free; you pay for legalisation, translation, Nuffic and the NT2 exam

Worth knowing: Ergotherapeut is an Article 34 profession: the title is protected but there is no BIG-register entry, which confuses many applicants.

Confidence: medium. We could not fully confirm every detail here from official sources. Check before relying on it.

Step by step 5 steps

  1. Check your route with the official advieswijzer tool at advieswijzer.bigregister.nl.
  2. Apply to CIBG for a verklaring van vakbekwaamheid using the general form plus the ergotherapeut annex, with legalised SA documents.
  3. Nuffic or SBB evaluates your diploma and subject list inside the CIBG procedure.
  4. Prove Dutch at CEFR B2 (Staatsexamen NT2) and the ability to read professional literature in English.
  5. After the CBGV advises and recognition is granted you may use the title ergotherapeut; in practice also join the Kwaliteitsregister Paramedici so insurers reimburse you.
Evidence and sources (4)

Demand evidence: No Dutch shortage list; ergotherapeuten use normal work-permit routes. No current UWV occupation data reachable. (limited evidence for this country) (source)

4 Germany Major requalification High demand

German at B2 is compulsory and your degree is compared with a 3-year vocational training, so top-up is likely.

Recognition
2/5 Substantial re-study, long supervised practice, scarce posts or a hard language barrier.
Time to practise
1-2 years including learning German, longer if an adaptation course is imposed
What it takes: exams, language test, cost, regulator
Exams
Kenntnispruefung, only if the authority finds substantial differences; the alternative is an adaptation course.
Language test
German at CEFR B2. The doctors' B2 plus C1 medical-German rule was not found to apply to occupational therapists.
Rough cost
Recognition fees vary by state: EUR 164 in Berlin; EUR 40-70 plus EUR 40 for the certificate in Bavaria, plus translations, German courses and any exam (Bavaria's Fachsprachpruefung costs EUR 350)

Worth knowing: A decision is due within 4 months of complete documents, but rules and fees vary across the 16 states. Your degree earns no extra status.

Confidence: medium. We could not fully confirm every detail here from official sources. Check before relying on it.

Step by step 5 steps

  1. Apply for Anerkennung to the authority of the German state where you plan to work, with certified German translations of your degree, curriculum, CV and clearances.
  2. Reach German at CEFR B2, confirmed as the requirement in Baden-Wuerttemberg, Hamburg and Berlin.
  3. The authority runs a Gleichwertigkeitspruefung against the German 3-year Ergotherapie Ausbildung; work experience can offset gaps.
  4. If substantial differences remain, take either a Kenntnispruefung or an Anpassungslehrgang of up to 3 years.
  5. Receive the Erlaubnis to use the title Ergotherapeut or Ergotherapeutin and start work.

4 United States Major requalification High demand

Blocked on a bachelor's alone: NBCOT requires a master's-level OT qualification before you may even apply.

Recognition
2/5 Substantial re-study, long supervised practice, scarce posts or a hard language barrier.
Time to practise
3-5 years if you must add a master's first; 9-18 months if you already hold one
What it takes: exams, language test, cost, regulator
Exams
NBCOT Occupational Therapist Registered (OTR) certification examination.
Language test
NBCOT sets no English test as the exam is in English. Individual state boards may differ.
Rough cost
OTED USD 850 plus exam USD 540, about USD 1,390, excluding master's tuition and state licence fees

Worth knowing: Occupational therapists are not on the US Department of Labor Schedule A shortage list, so there is no immigration shortcut either.

Step by step 5 steps

  1. Know the blocking rule: NBCOT requires an entry-level master's in OT, or a bachelor's plus a post-professional master's or doctorate, or a master's in a related field.
  2. Complete a qualifying master's-level qualification on top of your BSc, plus at least 960 hours of fieldwork.
  3. Apply for OTED (Occupational Therapist Eligibility Determination) with primary-source transcripts, syllabi, a programme director form and HPCSA verification.
  4. Pass the NBCOT Occupational Therapist Registered (OTR) certification examination: 4 hours, 180 items, computer-based.
  5. Apply for a licence in the state where you will work, and separately obtain employer-sponsored work authorisation.

4 Canada Exams or bridging High demand

Your SA bachelor's is the right level, but expect a long assessment plus a national exam, about CAD 6,000.

Recognition
3/5 Licensing exams, an adaptation period or a conversion course. Typically 6 to 24 months.
Time to practise
18-30 months
What it takes: exams, language test, cost, regulator
Exams
NOTCE (two 100-question parts, about 70 percent pass mark, three attempts, two sittings a year), plus the JKAT and competency assessment inside SEAS.
Language test
IELTS Reading 7.0, Writing 6.5, Listening 7.0, Speaking 6.5 or equivalent; exemption is possible if the OT programme was taught entirely in English or French (classroom instruction, written work and fieldwork).
Rough cost
SEAS CAD 4,120 plus NOTCE CAD 755 plus provincial fees; roughly CAD 5,900 in Ontario before WES and extras

Worth knowing: Quebec is completely separate: no SEAS or NOTCE, an OEQ equivalence review, and French including the OQLF exam for a permanent licence.

Step by step 5 steps

  1. SEAS Step 1: academic credential assessment with a WES ICAP report; the benchmark is a Canadian bachelor's degree in OT, which your SA degree matches.
  2. SEAS Step 2: language readiness assessment, profession-specific assessment of your curriculum and fieldwork, and the online Jurisprudence Knowledge Assessment Test.
  3. SEAS Step 3: a 6.5-hour remote interview-style competency assessment; the certificate is valid 3 years.
  4. Apply to the provincial regulator, which grants exam eligibility; provisional supervised practice may be possible meanwhile.
  5. Pass the National Occupational Therapy Certification Examination (NOTCE), then register provincially.

5 Australia Recognised, paperwork only High demand

No exam: a portfolio assessment by the OT Board, and OT is short-staffed in every state and territory.

Recognition
4/5 Mutual recognition or an assessment plus registration. No major exams.
Time to practise
4-8 months if substantially equivalent; 12-20 months if supervised practice is required
What it takes: exams, language test, cost, regulator
Exams
None. A desk and portfolio assessment, plus supervised practice if the Board requires it.
Language test
IELTS Academic overall 7 (L7, R7, W6.5, S7) or OET, PTE or TOEFL equivalent from 23 April 2026; valid 2 years.
Rough cost
OTC migration skills assessment AUD 800; OTC Assessment of competence AUD 1,500; OT Board qualification portfolio assessment AUD 688; Ahpra application AUD 250 plus registration AUD 138 (from 1 October 2026)

Worth knowing: The assessment model changed in October 2025 and transitional rules apply. Ahpra fees are revised annually.

Step by step 5 steps

  1. Sit an approved English test: South Africa is not a recognised country under Ahpra's English standard (IELTS Academic overall 7 or equivalent).
  2. Submit the Portfolio for the assessment of non-approved qualifications (PNAQ-96) to the OT Board, which has assessed qualifications itself since 27 October 2025.
  3. Receive an outcome: substantially equivalent leads to general registration, while 'relevant' leads to limited registration plus a competence assessment.
  4. Apply for registration with an international criminal history check, recency of practice evidence and professional indemnity insurance.
  5. If graded 'relevant', complete the OTC Assessment of competence: supervised practice, generally six months full time. Get an OTC skills assessment for the visa.

5 New Zealand Open door High demand

New Zealand names South Africa in a fast-track pathway: no exam, usually no English test, fast residence.

Recognition
5/5 Unregulated there, or the South African qualification is accepted as it is.
Time to practise
3-6 months
What it takes: exams, language test, cost, regulator
Exams
None.
Language test
South Africa is a Recognised Country in OTBNZ Policy 704, so an SA-trained applicant normally needs no test.
Rough cost
Registration application NZD 1,265 plus an Annual Practising Certificate of NZD 588

Worth knowing: OTBNZ states a WFOT condition for UK, Irish and Canadian applicants but not explicitly for South Africans; confirm your programme's standing.

Step by step 5 steps

  1. Hold current HPCSA registration; South Africa is one of four countries in OTBNZ's Abridged Overseas Qualified Pathway.
  2. Apply in the OTBNZ portal with two certified IDs, certified degree and transcript, three references (one from an OT), police clearances and a CV.
  3. Complete the Nga Paerewa Te Tiriti eLearning course and the Desktop Competence Self-Assessment.
  4. OTBNZ decides within about 6 weeks of a complete file; then pay for an Annual Practising Certificate.
  5. Complete the induction and orientation recertification programme for practitioners new to New Zealand.

4 United Arab Emirates Exams or bridging High demand

2 years' experience as an occupational therapist first; HPCSA-registered applicants are normally exempt from the licensing exam.

Recognition
3/5 Licensing exams, an adaptation period or a conversion course. Typically 6 to 24 months.
Time to practise
6-12 months once experience is met (estimate)
What it takes: exams, language test, cost, regulator
Exams
Usually none: the April 2025 PQR exempts holders of a valid South African licence and good standing if they qualified recently (within 3 years in DHA's copy of the PQR, 2 years in the DoH Abu Dhabi copy) or have no gap in practice since their last job. Otherwise the authority's occupational therapy licensing exam (computer-based, via Prometric for DHA).
Language test
English; the PQR says all licensing exams are in English. No Arabic requirement was found for licensing.
Rough cost
DHA credentialing fee AED 200, plus separate DataFlow verification and Prometric exam fees not published on the pages checked; total unknown

Worth knowing: The exam exemption does not remove the 2-year experience rule; whether community service counts toward it was not confirmed. Each authority may add requirements, and a long gap in practice removes the exam exemption. Visas are employer-tied and do not lead to PR or citizenship; the Golden Visa is the long-term exception.

Confidence: medium. We could not fully confirm every detail here from official sources. Check before relying on it.

Step by step 5 steps

  1. Register with the HPCSA and gain experience: the April 2025 PQR asks an occupational therapist for a bachelor's degree in occupational therapy of at least 3 years and 2 years of experience after qualifying.
  2. Choose the emirate you will work in (DHA for Dubai, DoH for Abu Dhabi, MOHAP for the rest) and apply online (Sheryan in Dubai, TAMM in Abu Dhabi).
  3. Have your degree, internship, experience letters, HPCSA or council registration and good standing verified by DataFlow (primary source verification).
  4. Only if the exam exemption does not apply: pass the authority's computer-based occupational therapy exam in English (DHA uses Prometric); three attempts are allowed.
  5. Get a job offer: the registration only becomes a licence when a UAE health facility activates it, and that employer also sponsors your visa.
Evidence and sources (7)

Demand evidence: Abu Dhabi's 10-year Golden Visa for Elite Specialists in Health Fields is open to holders of an Abu Dhabi Department of Health practising licence and a bachelor's degree, with Department of Health endorsement; elsewhere in the UAE occupational therapists are licensed health professionals on an ordinary employer visa.
